It does not solve bad staffing or make charting magically quick, but it keeps me from borrowing vague phrases just to get through the screen.

After three decades, I needed the reminder that longer is not automatically stronger. I removed the background that did not serve the timeline. The note became shorter and easier to follow.

Apparently my case notes were turning into proof that I had made a lot of phone calls. The actual status was buried. I now close with three things: why I contacted them, what changed, and who owns the next step. Less padding. Easier handoff.
